Job Description

HealthWorks Medical, LLC is looking for a self-motivated and team-oriented Nurse Case Manager to join our HealthWorks Marine Medical Management team. Hours are typically Monday through Friday 8:00 am until 5:00 pm (with telework options).                                                                                                                    Some early morning hours and some late afternoon short-notice overtime is also a possibility.

 

Job Description Report:

•  Acts as patient advocate in delivery and coordination of patient care.


•  Provides case management to a group of patients providing specific assessments according to protocols and procedures


•  Able to manage a large number of patient’s cases with high energy and organization


• Collaborates with other health care providers and Medical Director regarding patient needs.


•  Review and assess medical records to determine employee’s ability to perform job duties


•  Respond to workplace injury or illness by triaging initial telehealth call and providing follow up care.
