Experience as CNA/HHA; Indiana Nurse Aide certification preferred; COVID vaccination; driver license and auto insurance; pass background, TB, and CPR tests.

Hospice RN Case Manager Plymouth

Hospice RN Case Manager

Registered Nurse with Indiana RN license; hospice case management; perform patient/family assessments; develop care plans; collaborate with interdisciplinary team; weekend rotation.

Indiana RN license; minimum 4 years in quality assurance and certification; supervisory experience; OASIS and ICD coding preferred; strong organizational and communication skills.

Registered Nurse Hospice Inpatient Unit PW Nights

Indiana RN license; hospice/home health experience preferred; CPR; valid driver's license and auto insurance; pass pre-employment checks.

RN Hospice Inpatient Unit 3pm-11pm

RN Hospice Inpatient Unit

Registered nurse with Indiana license; CPR certification; driver’s license; pass drug/background checks; comply with health immunization requirements; willing to work 3pm-11pm in inpatient hospice unit.

Hospice Social Worker LSW 

Social Worker

Hospice social work role requiring Indiana/Michigan LSW licensure, willingness to travel, and weekend rotation.

Director of Finance

Bachelor’s degree in accounting; five+ years accounting and supervisory experience; healthcare/finance background preferred; CPA preferred.
